Drs. Drew Scoles and Jeremy Wolfe describe the case of a 68-year-old patient with a history of multiple intraocular surgeries who was noted to have a tilted IOL after a "modified Yamane" IOL rescue. It has been noted that such tilts are often caused by a loss of the IOL's structural integrity at the optic-haptic junction. After first placing iris hooks for better visualization and rotating the IOL back into the correct position, the surgeons lasered the optic-haptic junction to lock in the IOL. The IOL was noted to remain stable both intra- and postoperatively, and the patient's vision improved from 20/400 to 20/60.
