A 60-year-old female underwent pars plana vitrectomy and membrane stripping for visually significant epiretinal membrane. A xenon light source was used during surgery. No adjuvant, such as indocyanine green, was used to stain the membrane.

Following surgery, visual acuity declined to 20/200 in the operated eye. This fundus autofluorescence image, obtained approximately two months after surgery, demonstrates pigment mottling in the region of the membrane peel.

Phototoxicity is an infrequent complication of intraocular surgery. In this case, it may have been associated with the operating microscope, the intraocular light source or both.
