  • Login using your Academy login credentials.
  • Click Report CME Credit on the top right menu option.
  • Search for the media type "Live event".
  • Select the 2014 CODEquest course that you attended

JCAHPO CE Credits 

Ophthalmic Allied Health Professionals receive 4 JCAHPO “A” CE credits for completion of this course.

  • Step 1: Complete Non Physician/Staff Evaluation within 30 days of attending CODEquest.
    Be sure to include your e-mail address when completing the evaluation.
  • Step 2: After completing the evaluation, a confirmation e-mail will be sent to you with your downloadable certificate.
