CLAIMS REPORTING (WITHOUT REGISTRY)

REPORTING PROCESS 20 cataract patients. Of the 20 patients, a majority, or at least 11, must be Medicare Part B patients. Requires Web-portal entry of patient data to meet nine PQRS measures The registry can extract data from EHR systems for reporting on the nine clinical quality measures for meaningful use, which can also meet the PQRS requirements in 2014. Requires reporting in real time from your office.
REPORTING TIME FRAME Approximately April through Sept. 30, 2014. Enter data into the IRIS Registry online portal anytime before Jan. 15, 2015. Can be uploaded by the IRIS Registry any time before Jan. 15, 2015. Must be submitted on the same CMS 1500 form as the associated exam.
IRIS REGISTRY DEADLINES Aug. 1: Submit signed IRIS Registry agreements.

Sept. 30: Provide presurgical forms and perform cataract surgery on 20 patients meeting selection criteria.

Oct. 21: Submit signed consent, allowing data to be sent to CMS.

Jan. 15, 2015: Submit all data to the IRIS Registry.
Oct. 31: Submit signed registry agreements, including signed consent allowing data to be sent to CMS.

Jan. 15, 2015: Submit all of data to the IRIS Registry.

June 1: Submit signed IRIS Registry agreements.

Aug. 1: Map your EHR to the IRIS Registry.

Sept. 1: Submit signed consent, allowing data to be sent to CMS.

1 Participation will be fee-free in 2014 and 2015 for the first 4,000 participants who sign the agreements and meet the IRIS Registry's data submission requirements.
