    This video shows a case of a 69-year-old hyperope with severe Crystalens Z syndrome that was not improved with YAG posterior capsulotomy combined with YAG relaxing incisions of the anterior capsulorrhexis. The patient has severe astigmatism and decreased BCVA, with an intolerable anisometropia. An IOL exchange is attempted.
