Traditional options in managing failed trabeculectomy (bleb needling, revision, additional incisional surgery, and tube surgery) have relatively high failure and complication rates. The authors implanted the XEN Aquesys subconjunctival shunt in a patient with prior failed trabeculectomy with complete success (no drugs). The stent was close to the scarring of the previous trabeculectomy and became partially exposed. The conjunctiva over the exposed area was removed and replaced by an amniotic membrane and a conjunctiva autograft. Six months after surgery, IOP was well controlled (maximum 13 mm Hg, minimum 6 mm Hg, average 7.63 mm Hg). The surgical technique and clinical results are extensively presented in this video.
