Modified Macular Hole Surgery without Intraoperative Fluid Air Exchange or Gas Tamponade

This video demonstrates an effective technique of achieving macular hole closure without intraoperative fluid air exchange or gas tamponade and postoperative prone positioning. The authors performed pars plana vitrectomy with posterior vitreous detachment induction and double-staining of the internal limiting membrane (ILM) using autologous blood and brilliant blue G as well as ILM-peeling with removal of redundant ILM after retaining a flap. The ILM flap was gently nudged into the hole's edges and sealed with small bolus of autologous blood. Serial images at days 1, 2, and 7 show progressing closure of the hole and visual improvement.
