Not All Is Lost After a Posterior Capsular Tear

This video demonstrates successful management of posterior capsular rent in a series of patients undergoing phacoemulsification, establishing guidelines on how to manage such cases. Posterior capsular rents were noticed during various stages of nucleus removal and cortex removal as well as IOL implantation. When present, vitreous was stained with triamcinolone. Bimanual limbal anterior vitrectomy was performed when indicated. In most cases, a 3-piece hydrophobic acrylic IOL was placed in the sulcus, with posterior optic capture through the rhexis. All patients regained normal vision.
