This video demonstrates surgical pearls in type 1 big bubble (BB) formation in pre-Descemet stripping endothelial keratoplasty (pre-DSEK) and correlates these findings with optical coherence tomography (OCT) imaging and histology. Sixteen human corneal specimens consented for research and unsuitable for transplantation were obtained from an eye bank. After intrastromal air was injected into the donor cornea, a BB was formed. Key illustrated points include the use of viscoelasticum to increase the bubble size and seal perforations and the different ways to prevent a type 2 or type 3 BB formation. Histology and OCT confirmed the anatomy of the 3 distinct BB types and demonstrated pneumatized stroma in the setting of a failed BB.
