Dr. Soosan Jacob shows how she performed cataract surgery in a patient with an intraocular foreign body. After suturing the corneal wound and staining the capsule with trypan blue, she performs the capsulorhexis with difficulty. After phacoemulsification, she finds a large central posterior capsule rupture and vitreous loss from the corneal wound. She uses an AC maintainer to prevent the globe from collapsing, and brings the metallic foreign body into the anterior chamber with glued IOL forceps for safe removal using an intraocular magnet. A 3-piece IOL was implanted in the sulcus. At 1 day postop, the patient’s BCVA was 20/30.
