Dr. Fernando González del Valle shows a new cow hitch suture technique to manage late in-the-bag IOL dislocation. With just a simple intraocular cow hitch knot, he says the dislocated IOL can be repositioned while preserving the capsular bag and ocular anatomical compartmentalization. The technique consists of grasping and binding both haptics of the IOL (single or 3-piece IOL) and then suturing beneath a deep scleral flap using a 10-0 polypropylene suture and 2 disposable 25- or 23-g vitreoretinal micro forceps.
