Dr. Virgilio Centurion presents a case in which aggressive BSS injection during corneal infiltration caused the capsular bag to explode at the end of what was routine cataract surgery. After corneal infiltration of both incisions, Dr. Centurion gets a surprise as the IOL sinks into the vitreous cavity. The reaction happened so quickly it was impossible for him to see at the time, but he believes some of the BSS got into the capsular bag, causing the IOL to push forward until pressure got so high the posterior capsule exploded and the IOL fell into the vitreous cavity, nicking the retina, leading to posterior segment vitreous hemorrhage at 8 days postop. He said he learned that during corneal infiltration, it’s best “to be kind, soft and never rough.”
