History of the Lasso Suture

Dr. Robert Osher shares 2 techniques for exchanging an IOL, as well as the history of the lasso suture for repositioning an IOL. In the first case, he notices a deep scratch on the optic after implanting the IOL. He first enlarges the incision to 3.5 mm and makes a second stab incision across from the main incision. He then takes the distal haptic out of bag and places a hook underneath the IOL optic. He folds the optic over the hook, rotates his hands 90 degrees and explants the IOL. In a second case, the IOL injects into the anterior chamber – in 3 pieces. Again, he enlarges the incision to 3.5 mm, but this time he makes a radius incision in the optic, rotates it and makes a second radius incision, extracting the resulting 90 degree quadrant through the incision, making the remaining IOL easier to remove.
