    Iatrogenic Zonular Dialysis

    After uneventful phaco, a hydrophilic acrylic IOL was injected but suddenly shot out like a bullet, causing  a 180-degree zonular dialysis with the capsular bag still intact. OVD was injected and the wrinkled capsular bag re-inflated. A capsular tension ring was inserted through a side port to stretch the capsular bag, and a capsular tension segment was used to fixate the capsular bag to the sclera. After suturing of the scleral pocket, a 1-piece hydrophobic acrylic IOL was injected into the capsular bag, and the IOL and capsular bag were recentered.
