Dr. Suven Bhattacharjee shows how he uses his hexagonal pupil expansion ring in a patient with an advanced cataract and a small pupil, demonstrating the careful maneuvers required to accommodate a small anterior chamber and corneal scarring. As the surgery proceeds, signs of iris prolapse began to appear at the incision. Using a spatula, he enters the eye through the side port incision and manages to successfully drag the iris back into the anterior chamber. Despite the complications, the cornea is clear and the pupil, unharmed.
