Drs. Chakrabarti show how they repositioned a decentered IOL. Despite a series of unexpected complications, the patient had a good outcome following phacoemulsification. But at 4 weeks, the patient presented with monocular diplopia, severe dysphotopsia and decentration. They begin by creating 2 new paracenteses and injecting a dispersive OVD in anterior chamber to tapenade the area of posterior capsule rent. Each haptic is carefully brought into the anterior chamber and the decentered IOL is flipped back with the help of 2 Sinskey hooks. Following surgery, the IOL was stable and the patient achieved a BCVA of 6/12.
