Is a multifocal IOL recommended when macular pucker is present?

Question:
Can I have multifocal IOLs implanted during cataract surgery with a macular pucker present?
Answer:
This would not be recommended as the macular pucker (a layer of scar tissue in the back of the eye) creates optical aberrations in your visual system and these will be exaggerated by the multifocal lens, effectively making your vision worse rather than better. I would suggest a traditional monofocal lens, a toric lens if you have much astigmatism, or a Crystalens accommodating lens if you really want a range of vision—just realize that the pucker may hold back your best corrected vision (you may never get to 20/20, for example).
