Overview
You are eligible to apply for membership as an Active Fellow or Osteopathic Fellow if:
- You hold a degree of Doctor of Medicine, Doctor of Osteopathy or an equivalent medical degree; and
- You hold a valid and unrestricted license to practice medicine in the United States or the country in which the practice of medicine is regularly conducted; and
- You have been certified by the American Board of Ophthalmology, American Osteopathic Board of Ophthalmology or you are a Fellow of the Royal College of Physicians and Surgeons of Canada.
The Board of Trustees may waive the requirements that a candidate for Active Fellowship be licensed to practice medicine or be certified by a medical specialty board, if the physician has contributed significantly to the progress of medicine in its relation to diseases of the eye or related structures; or a physician who holds a degree of Doctor of Osteopathy, who holds a valid and unrestricted license to practice medicine in the United States, Canada, or the country in which the practice of medicine is regularly conducted, and who has been certified by the American Osteopathic Board of Ophthalmology, shall be eligible to apply for membership as an Active Osteopathic Fellow.
To apply for membership:

- Print the application.
- Obtain all necessary signatures.
- Sign the application and include the application fee payment.
- Mail or fax the completed application to Member Services.

Application Fee: $825 (USD) effective Jan. 1, 2008.
