Why Use the Chart-Auditing Service?
Audits are a matter of when, not if. Prepare now to ensure your records are in the best shape possible by having the Academy’s coding experts conduct a thorough audit of your chart documentation.
The Academy’s Chart-Auditing Service provides you with:
- Guidance for corrective and preventative action
- Timely results and recommendations via a one-on-one teleconference and a final written report
- Peace of mind
The fee is $600 per set of 10 chart notes.
- A single chart note is identified as a single patient encounter for one date of service.
- We review in sets of 10 single chart notes.
- You can choose one or more sets of charts for review.
- The chart notes can be from one or multiple physicians in your practice.
- Indicate your choices on the online submission form.
How it works
Step 1: Submit your order online. Within 10 business days you will be contacted by Academy staff to discuss contract terms.
Step 2: Once you have reviewed and approved the terms, you will receive a final Chart-Auditing Agreement and invoice. Please sign and return the agreement via email, fax, or mail, and send in your payment.
Step 3: Choose the chart notes that you want to have reviewed.
Step 4: Completely de-identify all records to ensure conformance to HIPAA standards. (If paper records, provide a signature log with the typed name, signature, initials and title of all physicians and staff who document the medical records).
Step 5: Send your de-identified records by one of two methods listed below:
- Scan and send them via email to email@example.com, or
- Photocopy and send them via FedEx*, or UPS* to:

Step 6: Within 21 business days, Academy staff will contact you to set-up a one-hour telephone appointment with our coding experts to review the findings of your audit outcome. Upon completion of your one-on-one telephone consultation, you will receive a final summary evaluation.

Coding is a complicated process involving continually changing rules and the application of judgment to factual situations. The Academy does not guarantee or warrant that either public or private payers will agree with the Academy’s information or recommendations. The Academy shall not be liable to you or any other party to any extent whatsoever for errors in, or omissions from any such information provided by the Academy, its employees, agents or representatives. The Academy’s sole liability for any claim connected to its provision of coding information or services shall be limited to the amount paid by you to the Academy for the information or coding service.
