HCPCS Code for Avastin
Question: What HCPCS code do I submit for Avastin?
Answer: Depending on the payer and site of service, bill for Avastin with one of the following HCPCS codes: C9257, J3490, J3590, J7999, or J9035. There is not one single code that is recognized by all payers. The Academy provides a listing of MAC requirements. Commercial payer requirements vary. Practices must verify which HCPCS code is recognized prior to billing.
