    Question: What are the requirements to bill an “incident-to” service?

    Answer: 

    1. The ophthalmologist must first see and diagnose the patient.
    2. There must be a clearly stated written order detailing what components of exam and testing services are to be performed. No standing orders.
    3. Direct supervision is required. The physician needs to be on-site and available if necessary, but not in the room.

    All testing services are bundled with 99211, including refractions.
