Using Modifier –JW Correctly
Question: How do you document waste for a single-use vial? How is modifier -JW used properly?
Answer: Effective Jan. 1, 2017, CMS will require the use of modifier -JW to indicate the amount of drug wasted. This is a six-month delay from the original implementation date. Be sure to document any waste in the patient’s chart, along with what amount was given. When you submit the claim, submit the first line with the drug and amount given. Submit a second line with the drug and include modifier -JW plus what was discarded. Be sure to include the NDC information in box 19.
