Part of the EyeSmart Campaign

The sixth-annual Eye Injury Snapshot will take place from May 17 to 24, 2009. To participate, fill out a simple data form for each eye injury you treat that week. Data can be submitted two ways:

  1. Download and complete the data form (PDF 165K), then fax it back
  2. Use the online form at www.useironline.org/snapshot

All data must be submitted by June 2 for inclusion in the 2009 data set.

Image Submissions
Photos are not required as part of the report submission, but if you would like to provide an image, e-mail it to eyesmart@aao.org along with a signed patient consent form (DOC 50K). Injury photos may be used in an article in EyeNet magazine or another publication.

About the Project
The Eye Injury Snapshot is an annual study conducted by the American Academy of Ophthalmology and the American Society of Ocular Trauma to identify the nature and prevalence of eye injuries. It has been conducted each year since 2004.
