A 28-year-old Caucasian woman presented with decreased vision in her right eye, which had been troubling her for six months. Ocular and medical histories were unremarkable.

On examination, her visual acuity without correction was 20/25 in the right eye and 20/20 in the left. IOP was 13 mmHg in the right eye and 12 mmHg in the left. Fundus examination showed an optic pit along the temporal optic nerve head of the right eye with shallow macular detachment. The macular detachment was a 2 to 3 disc area extending from the optic nerve toward the fovea. Optical coherence tomography showed total retinal thickness to be 463 µm. Fluorescein angiography revealed hyperfluorescence in the area of the detachment.

The patient was treated with intravitreal gas injection and face-down positioning to flatten the detachment. This was followed with laser treatment to the retina near the optic pit. One week later, retinal thickness measured 400 µm.
