In this 1-Minute Video, Dr. Ahmed Tabl manages a full-thickness idiopathic macular hole with phacovitrectomy combined with inverted internal limiting membrane (ILM) flaps. After cataract surgery, Dr. Tabl performed 23-gauge pars plana vitrectomy. He used a blunt scraper to create the first ILM flap, created an inferotemporal gutter using end-gripping forceps, and then finished creating the inverted flaps using a scraper. The patient’s vision improved from hand motion to 6/24 at 1 month and 6/18 by 3 months.
