    In this video, Dr. Morgan Micheletti shows his modification to the "twist and out" technique for IOL exchange originally developed by Dr. Rahul Pandit. This modification uses a 25-gauge cannula bridged across the anterior chamber through 2 side ports, replacing the need to hold a spatula in the off hand. The off hand is then free to perform a two-handed twist of the forceps, which enables a simple and controlled rolling of the IOL. The technique provides tighter wrapping of the IOL around the forceps, which can reduce wound stretch or allow for lens removal through a smaller wound.
