Diffuse Zonulopathy During Capsulorrhexis and Nuclear Emulsification

This presentation reviews a case of zonular loss and fluid misdirection. When performing capsulorrhexis and nuclear emulsification, Dr. Uday Devgan noted diffuse zonulopathy in the patient, who had previous orbital trauma. During cortical cleanup, Dr. Devgan encountered shallowing of the anterior chamber and a large temporal zonular dialysis. The audience and an expert panel examine ways to proceed: for example, cautiously continuing irrigation and aspiration (I/A) of the cortex or implanting a capsular tension ring before resuming I/A.
