Clear Opening in the Posterior Capsule After Manually Cracked Nucleus

In this presentation, experts discuss what to do when a posterior capsular rent appears early in a procedure. In this case, Dr. Julie Schallhorn and her resident began by sculpting a deep groove; however, they noticed a clear opening in the posterior capsule after the nucleus was manually cracked and the halves were separated. The audience and an expert panel examine next steps, including stopping phacoemulsification and injecting triamcinolone, elevating the heminuclei into the anterior chamber with an ophthalmic viscosurgical device, and rotating the nucleus and sculpting away from the defect.
