This presentation highlights the challenge of removing a posterior polar cataract while trying to correct refractive error. Dr. John Berdahl presents the case of a 41-year-old woman with nearly 4.00 D of with-the-rule astigmatism and a posterior polar cataract OD. An expert panel and the audience consider which intraocular lens (IOL) to use with this type of cataract (eg, monofocal high-powered toric IOL, light-adjustable IOL), as well as how to handle a rupture as the case progresses.
