This presentation features a complication of perforated sclera with choroid exposure. Dr. Elizabeth Yeu was performing an intraocular lens (IOL) exchange on a patient with a dislocated bag/IOL. While making scleral flaps for intrascleral haptic fixation of a posterior chamber IOL, one scleral flap incision perforated, resulting in gaping exposure of the choroid. Members of an expert panel discuss how they would continue, and Dr. Yeu details her approach and the importance of having lens backups.
