This presentation highlights a case of zonular dialysis with a deep and dense lens. Dr. Mitchell Weikert presents the operation on an eye with a brunescent lens that developed after pars plana vitrectomy; a 4-5 clock hour zonular dialysis was identified after removing one heminucleus. An expert panel and the audience consider how to proceed, including options such as inserting capsule retractors or capsular tension segments, inflating the capsular bag with a dispersive ophthalmic viscosurgical device, and prolapsing the heminucleus and performing supracapsular phacoemulsification. Dr. Weikert also describes his later use of a gonioscopy lens.
