    Since the introduction of reusable silicone irrigation and aspiration (I&A) tips in 2002, the incidence of posterior capsular rupture due to I&A has been exceptionally rare when these tips are used. This video reviews a series of cases of posterior capsular rupture due to silicone I&A tips that occurred in our surgical center over a 3-month period. Management of this complication is reviewed. A root cause analysis of these complications was performed. Improper handling and care of the tips was initially identified as the cause. Despite resolving these issues, additional cases of posterior capsule rupture due to these tips continued to occur. Scanning electron microscopy reveals the ultimate cause for these complications.
