A 52-year-old male with Down syndrome, cardiomyopathy and pulmonary hypertension had bilateral dense cataract surgery. General anesthesia was not possible, and the whole surgery was planned under intravenous midazolam. Hence we decided to do bilateral simultaneous cataract surgery by bilateral simultaneous cataract surgeons. The whole surgery was over in 4 minutes. The patient was very happy, with vision 6/6 postoperatively.
