In this case, presented by Dr. Holmes at Pediatric Ophthalmology Subspecialty Day 2016, a 70-year-old woman with 40-year history of vertical diplopia managed with vertical prism had undergone right superior rectus (RSR) recession and left inferior rectus (LIR) recession 1 year previously by a referring provider.
