    We present a modified technique of DSAEK in aphakic vitrectomized eye with corneal decompensation. Such cases are challenging in view of risk of posterior dislocation of donor lenticule and repeated globe collapse. An irrigation cannula was fixed at 3 mm from limbus. A multi-piece foldable IOL was implanted through a temporal limbal tunnel over fibrosed capsular rim. After Descemet's scoring, the posterior donor lamella was inserted with Busin glide and air was injected. The successful unfolding of donor tissue and maintenance of air tamponade was achieved due to positive posterior fluid pressure maintained by irrigation cannula at the pars plana.
