Traumatic retinal detachments (RDs) carry significant visual morbidity, and given wide variability in case presentations, the design of guidelines for surgical management is limited. The repair of a traumatic case secondary to a bilateral auto-enucleation attempt with open globe insult sustained during a schizophrenic episode is shown. The procedure addresses a macula-off RD detachment from a giant retinal dialysis with multiple interrupted retinal tears and incarcerated retina. The patient had delayed repair of the surgical eye shown because of prior open globe stabilization repair eight weeks prior to our surgery, along with a prioritized vitrectomy to the contralateral eye. A 23-gauge pars plana vitrectomy was performed with 360-degree relaxing retinectomy due to extensive proliferative vitreoretinopathy followed by direct perfluorocarbon liquid (PFCL) to silicone oil (SO) exchange to achieve anatomic flattening. The benefit of using PFCL for stabilization during retinectomy and preventing retinal slippage during SO delivery will be demonstrated.
