    What do you do when the power of the IOL is 60 or 62 D? When the eye is only 15 mm long? It is not only a matter of size; it is also a problem of space. We present the case of a 26-year-old woman with unremarkable systems review, high degree of hyperopia, refractive error of OD +13.50, OS +15.25 and a closed-angle glaucoma on full treatment but not controlled. The challenge was choosing the IOL power, which was the Hoffer Q for emmetropia 60.00 D OD and 62.00 D OS. The surgery was a challenge due to the lack of space, and the implanted IOL was of 50 D.
