    In this presentation, Dr. Thomas Samuelson shares a challenging case of an 81-year-old man with macular degeneration and pseudoexfoliation glaucoma. The plan was to perform phacoemulsification and Hydrus implantation. Dr. Samuelson placed a Malyugin ring and implanted the intraocular lens; however, the patient became agitated, and the eye became firm with shallowing of the anterior chamber. Experts discuss proposed options, including stopping the surgery temporarily or sending the patient home, removing the ring and ophthalmic viscosurgical device, and performing a pars plana vitreous tap.
