Dr. Norm Zabriskie explains how he decides whether to perform cataract and glaucoma surgeries alone or in a combined surgery, and then presents his evolved technique for combined phaco/trabeculectomy. For most cases of moderate to severe glaucoma, Dr. Zabriskie currently prefers a 2-site procedure: phacoemulsification through a clear corneal incision followed by trabeculectomy enhanced with MMC. For mild glaucoma with a target IOP in the upper teens, he prefers a combined procedure with trabectome. This approach has the advantage of leaving the conjunctiva pristine for future filtration surgery if needed.
