We will demonstrate with video 9 challenges that phaco surgeons confront and demonstrate pearls in their management. The specific challenges include phaco in the presence of a very shallow anterior chamber, small pupil and intraoperative floppy iris syndrome, hydroexpression of the lens, phaco in the presence of a small pupil fibrosed to the lens, 360-degree posterior subluxed cataract, phaco post–malignant melanoma excision, rock hard nuclei, iris neovascularization and bleeding, and management of soft nuclei for refractive lens exchange.
