Repeat Harada-Ito

In patients with acquired bilateral IV nerve palsies with combined vertical and torsional diplopia, surgery strategy should include addressing the torsional component if this is a barrier to fusion. If torsional diplopia recurs after a successful initial Harada-Ito (H-I) procedure, prisms alone will not be sufficient, and repeat surgery to correct torsion should be considered.
