    Subconjunctival scarring secondary to subconjunctival dissection can lead to filtration failure. This video describes a new technique for guarded filtration surgery, stab incision glaucoma surgery, which uses a single instrument (bevel-up keratome) to create a 2.8-mm conjunctival entry and corneoscleral tunnel in a single step while eliminating subconjunctival dissection. The ostium is punched, the conjunctiva closed, and a bleb is formed by hydrostatic pressure.
