    This video demonstrates a series of iatrogenic complications in a high myope undergoing phaco. An attempt to inject a capsular tension ring (CTR) tears the rhexis margin, and the 3-piece IOL unfolds in the reverse direction. To complicate matters, the IOL also flips in the anterior chamber, vaulting anteriorly. The haptics are tucked in the ciliary sulcus, the IOL is flipped back with a bimanual double Sinskey technique, and the lens is dialed until recentered.
