A 72-year-old female with Fuchs dystrophy and aphakia underwent a Descemet-stripping automated endothelial keratoplasty (DSAEK) OD that was complicated by intraoperative posterior dislocation of the donor graft. This video highlights the use of a 25-gauge vitreous cutter to aspirate the graft and elevate it to the iris plane; forceps through a corneal wound were used to grasp and explant the graft via a hand-to-hand technique. Repeat DSAEK was then performed with fresh donor tissue.
