Von Hippel Lindau cases usually have angiomas with feeder vessels that tend to bleed and can lead to extensive traction if untreated. The patient in this video had been managed over a few years with repeat laser treatments for multiple peripheral angiomas and eventually went on to develop a macular hole due to extensive traction around it. Surgical management was to release the traction and peel the internal limiting membrane, leading to a successful hole closure.
