    Dr. Liu presents a video of a 23 gauge pars plana vitrectomy with direct perfluoro-octane to silicone oil exchange for a total retinal detachment from a giant retinal tear. A thorough vitrectomy with particular attention to the vitreous base and horns of the tear are critical to surgical success. The advent of perfluoro-octane and advancement of surgical techniques has significantly improved the single surgery success rate for repair of retinal detachments from giant retinal tears. The value of adding a scleral buckle in giant retinal tears is still unsettled due to concerns of retinal slippage on a high buckle.
