Artificial Iris Placement During PK

Dr. Nicole Fram demonstrates placement of an artificial iris-lens combination during a penetrating keratoplasty (PK) in an aphakic patient with previous globe rupture. The size of the artificial iris is determined by measuring the white-to-white corneal diameter and cutting the prosthesis to 1 mm less. The IOL is sutured to the backside of the artificial iris with 9-0 polypropylene suture, and Gore-Tex sutures are “cow-hitched” into the IOL eyelets. The cornea is trephinated and a sclerotomy is made for knot placement before the diseased cornea is removed. To ensure proper centration and no irregular astigmatism is induced, the donor cornea is sutured into position before the anchoring knots are placed for the artificial iris-lens. The knots and suture ends are then buried in preplaced sclerotomies.
