Canaliculotomy for Pyogenic Granuloma Canaliculitis

Dr. Richard C. Allen demonstrates treatment of upper eyelid canaliculitis in a patient with a history of irritation and discharge. Examination showed purulent material expressed from the upper right punctum. To start, a punctal-sparring incision is made medial to the punctum, and 2 cotton-tipped applicators are used to express a canalicular stone, which is checked for a foreign body and sent to the pathologist. Additional material expressed from the canaliculus is consistent with a pyogenic granuloma. The incision is extended medially to examine for a retained foreign body. Finally, punctal dilation and lacrimal irrigation are performed to ensure patency.
