Dr. Avner Hostovsky demonstrates a combined flanged and sewing machine technique for repair of iridodialysis, which had persisted for more than 6 hours following cataract surgery in an elderly patient. With a modified sewing machine technique, the needle passes through the iris and out 2 mm behind the limbus. Using the flange technique to make sure that the suture stays in place, he holds the suture in one hand while passing the needle through the iris, again 2 mm behind the limbus. The resulting loop holds the iris to the sclera on the inside. The flange technique ensures that the suture stays in place, which can be seen buried under the conjunctiva.
