Cataract surgery may have grown by leaps and bounds, but couching is still performed in some parts of the world, as this video shows. The technique involves using a sharp instrument to pierce the eye at the limbus. The opaque lens is then dislocated from the central visual axis and pushed down into the vitreous cavity through the pars plana. Couching will become obsolete only when modern, high-quality cataract surgery becomes accessible and available to everyone across the globe.
